Forms Certificate Kit
Every Google Form response becomes a personal PDF certificate made from your own Google Slides design, saved in your Drive and emailed from your account. QR code to a verification page, expiry dates and reminders, revoke and reissue, designs per course and language, CSV import, dashboard and ZIP download. No branding, no subscription.

Buy on Gumroad €19, one payment. No subscription.
For trainers, teachers, schools, associations, webinar hosts and event organisers who hand out certificates and are tired of copying names into a template.
How it works. Link your form to a spreadsheet, paste one script into it (Extensions > Apps Script), and a Certificates menu appears. Point it at a Google Slides design with fields like {{Name}}, {{Course}} and {{Date}}. Three ready designs (classic, modern, minimal) are included, or use your own.
Issue - Your design, no branding. Your own Slides file: your logo, your colours, your wording. - Pass mark for quizzes, automatic issuing for every new response, or one click for all waiting responses. - Designs per course and per language, each with its own email text, file name and validity. - No form needed: import a CSV list from your Drive and issue from it. - PDFs in your Drive with a link per row, email with the PDF attached, an optional PNG image for social media, and a CC column for managers.
Verify - Unique certificate numbers and a QR code on the certificate that opens its verification page. The page says whether the certificate is valid, expired or revoked, and shows only the name, course and dates. A JSON answer is available for your website or LMS. - The QR code is made inside the script; no outside QR service sees your data.
Manage
- Expiry dates with reminder emails before a certificate expires.
- Revoke a certificate (the page then says so) and reissue with the same number and date after a correction or a new design.
- A dashboard tab with counts per course and month, pass rates and certificates expiring soon, a ZIP download of all PDFs, and a {{LinkedIn link}} that lets people add the certificate to their profile in one click.
Google's sending limits apply. Through Apps Script, a free Gmail account can email about 100 recipients a day and a Google Workspace account about 1,500. When the limit is reached, the kit waits and continues by itself.
Honest limits. Designs are made in Google Slides, not in a drag-and-drop editor. There are no digital badges, recipient wallet or share analytics. The verification page runs as a Google Apps Script web app, so its address is long and Google shows a small notice at the top. Each certificate takes a few seconds to make.
Private by design. The script runs in your own Google account. Names, emails and scores never pass through our servers or anyone else's.
Use it for as many forms and courses as you like, including for your clients.
Test status
Uses only Google services (Forms, Slides, Drive and Gmail).
